This position is onsite and involves working directly within patients' homes. As a member of our home care team, you will conduct evaluations and therapy sessions in various residential settings, ensuring a safe and comfortable environment for your patients. You will have the flexibility to manage your schedule, with typical working hours being Monday through Friday. • Conduct comprehensive evaluations of patients, reviewing medical histories, performing specific assessments, and determining customized treatment recommendations based on the evaluation results.
• Communicate evaluation findings and treatment plans clearly to patients and their families, ensuring their active involvement in goal-setting and progress assessments.
• Implement treatment protocols that align with evaluation outcomes, adapting techniques based on patient responses and evolving healthcare needs.
• Engage in consultations with other healthcare professionals, identifying and documenting any changes in patient conditions that may necessitate adjustments to their care plans.
• Facilitate thorough discharge planning and post-care follow-up, maintaining accurate and timely documentation that reflects the patient’s progress and outlines their continued care strategies.
• Utilize effective time management skills to meet productivity standards, keep a well-organized schedule, and remain punctual for all appointments and meetings.
• Play an integral role in departmental goal-setting and contribute to compliance with governmental and accreditation regulations.
• Pursue ongoing professional development, engaging in continuing education opportunities and serving as a mentor to students and new staff.
• Enhance the patient experience by exhibiting flexibility and responsiveness to their needs and concerns throughout the therapeutic process.

You will be servicing a diverse range of patients, many of whom may be dealing with chronic pain, recovery from surgery, or age-related mobility issues. Understanding the unique challenges and conditions faced by homebound patients is essential in designing effective, individualized treatment plans that cater to their specific needs.

• A Doctorate degree in Physical Therapy (DPT) is required, along with a valid state license to practice as a Physical Therapist in Florida.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association is mandatory.
• The role may include the necessity of state licenses specific to your locations of operation as deemed necessary.

• A minimum of 2 years of prior experience in physical therapy is preferred, but we welcome candidates who are newly graduated to apply, provided they possess a strong foundation in clinical skills and patient care.

The annual salary for this position ranges from USD 100,000 to 120,000, complemented by a comprehensive benefits package.
