Job description

TITLE:  DESK REVIEWER LONG TERM CARE ACCESS (LTCA)

DEFINITION:  Under the supervision of an Assessment Worker Supervisor in LTCA, this position is responsible for comparing annual redetermination data for Functional Eligibility Determination (FED) assessments completed by Service Coordinators at the Managed Care Organizations (MCO’s) to determine and ensure continued program eligibility for Community Health Choices (CHC) as well as recognizing any significant data change prior to the annual redetermination.

QUALIFICATIONS: 

Education and Experience:

 

  • High School Diploma or equivalent.
  • Some College Credits a plus.
  • Experience in social services and computer applications preferred.

Personal Characteristics:

 

  • Extremely well organized and detailed oriented with the ability to multi-task.
  • Excellent written communication and administrative skills
  • Strong computer skills including but not limited to Microsoft Office.
  • Ability to quickly acquire needed knowledge.
  • Manage a high volume of data and paperwork with maximum level of productivity.

    GOALS:
     
  • Successfully complete the required Pennsylvania Individualized Assessments (PIA) and FED training in the Aging Well Learning Management system to obtain a b-account and access to PIA.  
  • Ensure Desk Reviews are complete accurately and error free.
  • Maintain the highest level of professional ethics, continually seeking opportunities for increased knowledge, growth, and development within guidelines of departmental coverage and funding.

 

DUTIES AND FUNCTIONS: 

 

  1. Review a minimum of 50 Desk Review alerts per day using the Desk Review criteria set by Aging Well.
     
  2. Identify Desk Review alerts that meet criteria and complete documentation in PIA. Consult with Aging Well Quality Compliance and Supports Manager (QCSM) if there are any questions on whether a Desk Review should be completed.
  1. Maintain a Desk Review Tracking sheet for alerts that meet criteria for documenting required information on the sheet.  Forward completed sheet weekly to the supervisor. 
  1. Identify alerts that meet the priority criteria and complete a Desk Review template to attach in consumer’s PIA record and send a priority alert via email to the QCSM.
  1. Ensure all Desk Reviews completed are submitted and not left in progress prior to the end of the working day.  
  1. Complete a comprehensive End of the Day report that reflects priority cases, discrepancies, cases where more than one Desk Review was completed, and desk reviews that were unable to be completed. 
  1.  Attend staff, team, and in-service department and agency meetings as required.
  1. Performs other duties as assigned.
     
    PERFORMANCE: 
    Examples of performance measures:
     
  • Accurate and thorough completed Desk Reviews that is monitored through agency quality assurance checks and Aging Well quality assurance checks.
  • Adherence to LTCA policies and procedures.
  • Ability to work independently and autonomously
  • Timeliness of completed Desk Reviews.
  • Accuracy and completeness of data entry in PIA.
