Job description

Job Title: Manager Quality Program - Home Care
Department: Clinical Innovation
Location: Rochester Regional Health Home Care- 330 Monroe Ave Rochester, NY 14607
Hours Per Week: 40 hours
Schedule: 8:30am - 5:00pm
POSITION SUMMARY:
Manages quality including data collection, tracking, trending, and analysis, monitoring and evaluation of quality outcomes for publicly reported quality measures and performance improvement indicators, identification of performance improvement opportunities, and implementation of quality and performance improvement projects in direct collaboration with Quality and Education team members for RRH Home Care, Hospice, LHCSA, and Home Infusion Pharmacy. In conjunction with Senior Leadership: develops, implements, and communicates the Quality Assurance and Performance Improvement Plan. Prepares annual quality assurance reports and communicates to each of the professional advisory committees. Supervises and directs the work of the Quality and Education teams, and other support staff. Chairs performance improvement committees for appropriate business lines. Member of the affiliated advisory committees. Leads Clinical Education team to identify, develop, and validate specific strategies to meet the learning needs throughout the agency based on needs assessments, leadership, performance improvement results, customer surveys, data from external sources including benchmarks and course evaluation results. Supports the Orientation of new staff throughout the agency, meeting the learning needs of staff and promoting staff retention, quality health care, employee growth, and personal wellness. Evaluates the effectiveness of educational offerings.
The manager of the quality program ensures successful quality initiatives and performance improvement projects by effectively interacting with management and with the Compliance department. Leads or directs quality and education initiatives, ensuring implementation of task forces and action plans aimed at maintaining Rochester Regional Health Home Care and Hospice as a leader in Home Care quality. Contributes to NY State Department of Health and/or accrediting agency survey and the completion and implementation of any needed corrective action plan.
KEY RESPONSIBILIITES:
• Monitor, trend and report on outcomes based quality improvement and publicly reported outcomes performance for Senior Leadership and all staff.
• Initiate, direct and coordinate improvement activities.
• Educate staff on best-practice guidelines and initiatives related to quality initiatives
• Oversight of Clinical Education program including new-hire Orientation, continuing education development, student programs, annual mandatory education, and ongoing assessment of agency education needs.
• Assign performance audits to management and audit staff, educate on audit process. Work closely with data analysis team to analyze outcome of performance audit results, present results in high level format to Senior Leadership, at Quality Assurance Performance Improvement (QAPI) meetings, and affiliated advisory committees on a regular basis.
• Represent the health system at Regional, State and National meetings in regard to quality outcome improvement events when needed. Actively participate on National or State projects intended to improve quality outcomes.
• Collaborate with other clinical leaders on system-wide efforts regarding implementing best practice standards, methodologies and innovative technologies (ex. telemedicine units, lean six sigma projects) to enhance patient outcomes.
• Collaborate with system Hospital staff, Clinics and Physician practices when necessary to coordinate delivery of care that will best serve to improve patient outcomes.
• Prepare presentations regarding clinical outcomes, payment model associated outcomes, publicly reported outcomes, and other patient related clinical statistics.
• Work with finance team, data analysis team, and Senior Leadership to understand the financial impact of outcomes-based payment model measures and recommend improvement initiatives to increase scores.
• Chair the quarterly Quality Assurance Performance Improvement (QAPI) Committees for all affiliated business lines.
• Consistently demonstrates high standards of integrity by supporting the Rochester Regional Health mission and values and adhering to the Corporate Code of Conduct.
• Maintains high regard for member privacy in accordance with the corporate privacy policies and procedures.
• Maintains knowledge of all relevant legislative and regulatory mandates and ensures that all activities are in compliance with these requirements.
• Conducts periodic staff meetings to include timely distribution and education related to departmental and Ethics/Compliance information.
• Performs other functions as assigned by management.
• Regular and reliable attendance is expected and required.
PREFERRED QUALIFICATIONS:
• Master's degree in healthcare field preferred.
• 3-5 years of experience leading Quality Assurance/Improvement activities preferred.
• Prior management experience preferred
  • Experience with and knowledge of OASIS, ICD-10 coding, Home Health Star Ratings, Home Health Value Based Purchasing, and CMS Home Care and Hospice Quality Reporting Programs.

• Critical thinking, flexibility, and ability to serve as a change agent.
• Excellent communication skills, experienced with making presentations to both small and large audiences.
• Ability to work effectively with various levels of management personnel.
• Knowledge of Microsoft office programs: Excel, Word, and Power Point
• Strong analytical skills, experience with spreadsheets, graphs and Power Point presentations
Certification as a Professional in Healthcare Quality (CPHQ) and Certification Oasis Specialist- Clinical (COS-C) desired.
REQUIRED QUALIFICATIONS:
• Bachelor's Degree required.
• Minimum of 5 years of experience in area of clinical licensure (Registered Nurse, Physical Therapist, Occupational Therapist, Speech Pathologist, Licensed Master Social Worker.)
LICENSURE/CERTIFICATION:
Active NYS licensure as a Registered Nurse, Physical Therapist, Occupational Therapist, Speech Pathologist or Licensed Master Social Worker required.
