Catholic Charities Maine Behavioral Health Network in Portland, Maine has an opening for a Full-Time Nurse Care Manager.

Definition:   The BHN Nurse Care Manager Level I is a Licensed Practical Nurse (LPN) who functions as a core member of a collaborative care team including a Clinical Team Leader, Clinicians, Care Coordinators, Psychiatric Nurse Practitioner, Peer Support Specialist, healthcare consultants, and primary care partners. This role primarily supports psychiatric providers and Opioid Health Home programming under MaineCare Section 93, with a focus on medication-assisted treatment (MAT), care coordination, and direct nursing services.

Educational Qualifications: 

  • Licensed Practical Nurse (LPN), licensed by the Maine State Board of Nursing. License must be in good standing with the Maine State Board of Nursing.

Experience Qualifications: 

  • Minimum of two (2) years of direct service experience in a healthcare or behavioral health setting preferred.
  • Experience working with individuals with substance use disorder (SUD), Serious Mental Illness (SMI), or co-occurring conditions preferred.
  • Experience with electronic health records preferred.
  • Experience with injections such as Sublocade and Brixadi is required.

Personal Qualifications:

  • Demonstrates maturity, initiative, sound judgment, and creativity.
  • Strong communication skills (oral and written).
  • Ability to work within a multidisciplinary team providing person-centered care.
  • Valid driver’s license with clean driving record.

Physical Requirements: Requires the ability to sit, stand, and walk for extended periods; perform light lifting (up to 25 pounds); use a computer for documentation; and respond appropriately to patient needs, including occasional crisis situations in a behavioral health setting.
