Job Title: Health and Wellness Coach 
FLSA Status: Exempt
Department: Clinical  Supervisor:  
Position Summary 
The Health and Wellness Coach is responsible for developing and maintaining a wellness coaching relationship and assist patients through the process of actively working towards better health by providing support, encouragement, and education. 
Job requirements: 
•    Develop and maintain relationships with patients who are interested in health coaching/wellness programs and provide necessary wellness coaching to reduce or eliminate high-risk behaviors 
•    Develop and conduct lifestyle behavioral change programs, classes, workshops, presentations, health campaigns, and other programs 
•    Develop and distribute health education materials 
•    Take vital signs and participate in biometric screenings by interpreting results and making appropriate referrals 
•    Develop a customized care plan for program participants including goals and plan of action 
•    Write chart notes to document patient progress in both electronic medical record systems and wellness portals 
•    Assist in developing, implementing, evaluating, and marketing wellness education events, programs, and services 
•    Develop and maintain collaborative relationships with providers, nurses, fitness professionals, front desk staff, and patients 
•    Participate in meetings and training sessions as needed 
•    Stay current on research and wellness topics 
•    Additional responsibilities as assigned 
Skills/Knowledge: 
•    Knowledge of specific disease and lifestyle-related topics such as weight management, nutrition, stress reduction, smoking cessation, and chronic conditions. 
•    Excellent verbal communication and public speaking skills to promote health and wellness 
•    Adequate skills in health & wellness program management. 
•    Must be proficient with a computer and have a working knowledge of Word, Excel, Outlook, and PowerPoint. 
•    Must have excellent customer service skills and problem-solving skills. 
•    Ability to work well with a team and independently. 
•    Able to maintain client confidentiality.

Qualifications/ Experience
Bachelor's degree or higher in health promotion, health education, nursing, athletic training, nutrition, other health-related fields, or counseling
Registered Nurse or Registered Dietitian with 2-4 years of previous experience in a hospital or clinic setting is preferred
Two years' experience as a wellness coach preferred
CPR/AED certification preferred.
