Key Responsibilities
  • Conduct comprehensive evaluations of patients referred by physicians to develop tailored treatment plans that align with individual recovery goals.
  • Implement and monitor therapy interventions across a variety of specialties, including orthopedics, pediatrics, neurology, and sports medicine.
  • Provide skilled, evidence-based therapeutic care using procedures such as manual therapy, therapeutic exercise, and modalities to enhance patient function.
  • Educate and empower patients and their caregivers through hands-on training and continuous communication, ensuring they understand and are active participants in their recovery.
  • Utilize electronic health record (EHR) systems to document patient progress and treatment outcomes, complying with the practice's standards for documentation and billing.
  • Work collaboratively with a multidisciplinary team to coordinate care, sharing insights to maximize patient outcomes.
  • Meet and strive to exceed minimum utilization goals while contributing to the overall effectiveness of the clinic.
Patient Population / Client Focus

As a Physical Therapist at Upstream, you will work with a diverse client population that includes orthopedic patients recovering from surgery, pediatric patients with developmental needs, neurologically impaired individuals requiring specialized rehabilitation, and athletes seeking to enhance performance following injuries. Your role will include assessing each patient uniquely and crafting comprehensive care pathways that address their specific conditions and lifestyle goals.

Qualifications
  • A Bachelor's or Doctorate in Physical Therapy from an APTA or CAPTE-accredited program.
  • Current state license to practice physical therapy in New York.
  • Basic Life Support (BLS) Certification from the American Heart Association.
