We are currently seeking an experienced Registered Nurse (RN) or Registered Practical Nurse (RPN) with Advanced Foot Care Certification to provide professional foot care services to clients in their homes throughout the Greater Toronto Area.

This role offers flexibility, autonomy, and the opportunity to help clients maintain their independence and mobility through preventative and therapeutic foot care.

The Advanced Foot Care Nurse is responsible for providing comprehensive foot assessments, preventative foot care, treatment, education, and ongoing monitoring for clients receiving care in their homes. Working independently within the College of Nurses of Ontario (CNO) Standards of Practice, the successful candidate will promote foot health, reduce complications, and improve clients' overall quality of life.

This is a community-based position requiring travel between client homes.

Requirements

Key Responsibilities

Perform comprehensive foot assessments and develop individualized care plans.

· Provide routine and advanced foot care services, including:

· Nail trimming and reduction

· Thickened nail management

· Callus and corn reduction

· Basic skin and foot assessments

· Diabetic foot assessments and preventative care

· Client education regarding foot health and self-care

· Identify abnormalities requiring referral to physicians, podiatrists, chiropodists, or other healthcare professionals.

· Monitor clients for changes in condition and communicate concerns appropriately.

· Maintain accurate and timely documentation within The Care Company's electronic health record (AlayaCare).

· Obtain informed consent and complete all required clinical documentation.

· Collaborate with physicians, caregivers, and interdisciplinary healthcare providers as required.

· Maintain infection prevention and control standards during every visit.

· Follow all College of Nurses of Ontario Standards of Practice, organizational policies, and applicable legislation.

· Participate in quality improvement initiatives and ongoing professional development.

Qualifications

· Current registration in good standing with the College of Nurses of Ontario (RN or RPN).

· Valid Advanced Foot Care Certificate from a recognized educational provider.

· Previous experience providing foot care within community or home care settings is considered a strong asset.

· Knowledge of diabetic foot care and risk assessment.

· Strong assessment, clinical judgement, and critical thinking skills.

· Excellent interpersonal and communication skills.

· Comfortable working independently in clients' homes.

· Basic computer proficiency and ability to document electronically.

· Current CPR certification.

· Valid Ontario driver's licence (G2 or G) and access to a reliable vehicle.

Preferred Qualifications

· Experience working with seniors and individuals living with chronic diseases such as diabetes or vascular conditions.

· Knowledge of the Canadian Association of Foot Care Nurses (CAFCN) National Competencies for Advanced Nursing Foot Care.

· Previous community nursing experience.

· Experience using AlayaCare or other electronic health record systems.
