Job description

Medicaid Health Home Registered Nurse

$5,000 Sign-on Bonus

The Registered Nurse (RN) โ€“ Medicaid Health Home is responsible for delivering integrated, whole-person, person-centered care coordination services to Medicaid beneficiaries with Serious Mental Illness (SMI), Serious Emotional Disturbance (SED), Substance Use Disorders (SUD), and/or chronic medical conditions.

The RN supports compliance with Medicaid Health Home core service requirements, promotes quality outcomes, reduces avoidable hospitalizations and emergency department utilization, and ensures coordination across physical health, behavioral health, and social service systems.

Minimum Requirements:

Graduation from an accredited school of Nursing - Licensed as a registered nurse (RN) in the State of Michigan - Two (2) years of clinical nursing experience AND experience working with individuals with SMI, SUD, and/or chronic medical conditions AND knowledge of Medicaid regulations, managed care processes, and care coordination models.

Additional Requirements:

As THIRD PARTY insurance may be billed by this position, in accordance with the professional scope of practice, employee MUST MEET THIRD PARTY REQUIREMENTS. Must have a valid Michigan driver's license, current automobile insurance, and the use of a vehicle during work hours, including for the transport of clients; must be willing to work weekends, holidays, and irregular hours.

Essential Physical Demands:

  • Vision โ€“ Position requires the ability to visually assess details at both near and far distances, as well as to perceive depth and spatial relationships with or without reasonable accommodations.
  • Hearing โ€“ Position requires the ability to receive and respond to verbal communication with or without reasonable accommodations.
  • Lifting, Standing, and Walking With or Without Reasonable Accommodations:
    • Position requires the ability to lift up to 35lbs on an occasional basis
    • Position requires the ability to regularly stand for extended periods of time
    • Position requires the ability to walk at a minimum of 35% up to a maximum of 65%
  • Position will require the ability to occasionally stoop, bend, or reach to perform work tasks with or without reasonable accommodation.
  • Position must be able to perform tasks requiring manual dexterity, including the use of hands and fingers for handling, grasping, typing, writing, or operating equipment with or without reasonable accommodation.
