ComForCare Home Health Care - Hilton Head is looking for a dedicated and compassionate Registered Nurse (RN) to deliver exceptional in-home care to patients in the Hilton Head, SC area. Responsibilities:
  • Conduct comprehensive patient assessments and develop individualized care plans for home health clients
  • Administer medications, wound care, IV therapy, and other skilled nursing treatments in the home setting
  • Monitor and evaluate patient health status, documenting changes and communicating with physicians as needed
  • Educate patients and family members on disease management, medications, and home safety
  • Coordinate care with interdisciplinary team members including therapists, aides, and case managers
  • Ensure accurate and timely clinical documentation in compliance with home health regulations
  • Provide compassionate end-of-life and chronic disease management support as needed
Requirements:
  • Active Registered Nurse (RN) license in the state of South Carolina
