Key Responsibilities:

  • Develop wellness and preventive initiatives.
  • Spearhead health education classes, tapping into profound insights on complex medical conditions.
  • Contribute to the preliminary treatment scheme creation for all Healthcare Home participants.
  • Team up with professionals to design a targeted treatment strategy comprising healthcare objectives for individuals afflicted by dual chronic diseases.
  • Advise Community Support Specialists on patient-identified health complications.
  • Streamline hospital and medical facility admission and discharge proceedings for Healthcare Home beneficiaries.
  • Coach patients on complex health conditions, discussing medical diagnoses, therapies, and prescriptions.
  • Execute DMH Net Health technology projects and endeavors, such as CyberAccess and metabolic screening.
  • Track and relay performance metrics and triumphs using refined information systems engineered for nurses.

Education and/or Experience Qualifications:

  • Active RN or LPN license
  • Experience in direct patient care recommended
