In this role, you will have the opportunity to oversee the daily operations of the Healthcare Home (HCH) Team, making a difference in the lives of our clients. You will coordinate and track HCH clients, facilitating health education groups and activities that empower individuals. Your leadership will guide Nurse Care Managers, fostering a culture of collaboration and excellence. As the HCH Manager, you will manage and direct daily activities within a supportive environment, ensuring client needs are effectively met. You'll engage in various initiatives, driving quality assurance and improvement within the healthcare framework.

Key Responsibilities:

  • Oversee and supervise the daily operation of the HCH Team.
  • Responsible for coordination and tracking of HCH enrollees (new enrollees, discharges, transfers).
  • Supervise Nurse Care Managers and assign NCM caseloads in partnership with the Assistant Director.
  • Coordinate and/or facilitate health education groups and/or activities for clients, staff, and community.
  • Review, complete, and submit the monthly Attestation report in CyberAccess in a timely manner.
  • Serve as a Cyber Access Practice Administrator maintaining user accounts and monitoring agency usage and prescriber reports.
  • Coordinate management of HIT tools and initiatives, including CyberAccess usage, CMT reports, and hospital admission/discharges.
  • Oversee and/or facilitate staff training on Healthcare Home.
  • Participate in monthly TA calls for HCH Directors and attend state-required meetings and trainings.
  • Oversee Quality Assurance and Quality Improvement for Healthcare Home.
  • Partner effectively with collaborating service areas to meet client needs.

Education, Experience, and/or Credential Qualifications:

  • Current Registered Nurse (RN) in the state of practice.
  • Supervisory experience.

Additional Qualifications:

  • Successful completion of the Criminal Record and Child Abuse & Neglect Background check prior to beginning employment.
  • Completion of New Hire Orientation within 30 Days of employment.
  • All training requirements including Relias Learning within two (2) weeks of employment and annually thereafter.
  • Strong verbal and written communication skills, computer, and organizational skills.
