We are a locally owned and operated hospice and home health agency, celebrating our 27th year. We have an excellent reputation for providing very high quality, patient-focused care. We are looking for a qualified Hospice and Home Health RN Case Manager that shares our values and wants to assist in our vision to provide the excellent care that the community expects from us. We pride ourselves on very low employee turnover. We are looking for a qualified RN Case Manager who wants a long term position with a great company.

As a Home Health/Hospice RN Case Manager your duties will include but not be limited to:

  • Skilled nursing care including wound care, catheter care, venipuncture, observation and assessment, medication compliance and teaching, hospice assessments and other skilled care.
  • Create patient centered goals with patients, family members and providers
  • Assess, supervise and direct patient care
  • Develop plans of treatment
  • Ensure chart compliance with state regulations
  • Coordinate patient care with physicians and other providers

Hospice and Home Health are both challenging and rewarding! This job requires a person who will never forget our work requires a team effort. Other requirements include:

  • Licensed as a Registered Nurse by the State of Utah
  • Current Utah driver's license and automobile insurance
  • CPR certification
  • A minimum of 1 year nursing experience is required
  • Hospice and home health experience is strongly preferred but not required

Associate degree

Required experience:

Nursing: 1 year

Required license or certification:

Registered Nurse (RN)
