About This Role

As a Home Health Admit RN, you will provide and direct nursing care to patients in their homes, coordinating with healthcare professionals to ensure continuity of care. This role is ideal for those who are passionate about delivering high-quality patient care in a home setting.

What You'll Do
  • Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
  • Supervise care provided by home health aides and licensed practical/vocational nurses, providing instruction and assigning tasks.
  • Provide required supervisory visits.
  • Deliver high-quality clinical services within the scope of practice and infection control standards.
  • Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
  • Document patient visits per policy and payer requirements, syncing timely per LHC policy.
  • Make initial and/or comprehensive nursing evaluation visits to ensure home health eligibility.
  • Communicate relevant information timely and effectively with appropriate agency staff.
  • Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
  • Participate in on-call and weekend rotation as needed to meet patient needs.
Requirements
  • Education:
    • Current and unrestricted RN license in the state of practice
  • Experience:
    • 1+ years of Home Health experience preferred
  • Licenses & Certifications:
    • Current Driver's License and vehicle insurance required
    • Current CPR Certification or ability to complete within 90 days of hire preferred
