Position Overview
We are seeking an experienced Registered Nurse (RN) to provide high-quality, patient-centered care in a home health setting. The ideal candidate will be responsible for assessing patients, implementing individualized care plans, promoting health maintenance and disease prevention, and coordinating care with an interdisciplinary team. This role requires strong clinical judgment, excellent documentation skills, and experience with HomeCare HomeBase (HCHB) and OASIS.
Key Responsibilities
- Conduct comprehensive patient assessments and develop individualized plans of care.
- Observe, assess, plan, implement, and evaluate nursing care for patients with chronic, acute, or complex health conditions in the home.
- Provide ongoing case management to ensure patients receive appropriate, cost-effective care.
- Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate patient care and promote positive outcomes.
- Perform and document OASIS assessments and maintain accurate records using HomeCare HomeBase (HCHB).
- Monitor patient conditions, communicate changes to physicians, and coordinate timely interventions.
- Ensure medications are ordered, reviewed, monitored, administered, and discontinued according to physician orders and agency policies.
- Maintain accurate nursing documentation, including daily, monthly, quarterly, and annual clinical records as required.
- Coordinate healthcare appointments and follow up on recommendations to ensure continuity of care.
- Support safe transitions of care by communicating with hospitals, discharge planners, and other providers.
- Promote health maintenance, disease prevention, and patient education for individuals and caregivers.
- Supervise and provide clinical guidance to Licensed Practical Nurses (LPNs) and other support staff as needed.
- Train and educate new employees and unlicensed staff on health, safety, infection prevention, medication administration, and person-specific care needs.
- Ensure compliance with infection control standards and maintain a safe environment for patients and caregivers.
Qualifications
- Active Maryland RN License or Compact RN License required.
- Previous Home Health experience required.
- Strong working knowledge of HomeCare HomeBase (HCHB) and OASIS documentation.
- Excellent assessment, case management, and critical thinking skills.
- Strong communication and interdisciplinary collaboration abilities.
- Ability to work independently while managing a territory and patient caseload.
- Reliable transportation and willingness to travel throughout Wicomico, Worcester, and Somerset Counties.
