American Traveler is hiring an experienced RN for a Home Health Field RN position requiring HCHB and OASIS experience, with on-call rotation and a caseload of 4-6 patients per day.

• Home health field setting with patients seen in their own homes across a wide geographic service area in central Texas
• Geriatric patient population with a focus on in-home skilled nursing care, interdisciplinary care coordination, and plan of care development
• Average caseload of 4-6 patients per day, with a minimum of 6 points per day and 30 points per week required (each skilled nurse visit = 1 point)
• Charting via Homecare Homebase (HCHB) and OASIS, completed at the patient's home during the visit
• Monday through Friday schedule, 8:00 AM - 5:00 PM
• Weekends required as needed
• On-call rotation required, including approximately 1 night of call per week (details discussed at interview)
• Must be willing to travel up to 50 miles outside of the home base office
• Mileage reimbursed at the IRS standard rate, from first patient to last patient (commute to/from home not included)

• Active TX RN license or compact RN license in hand at time of consideration — no pending licenses accepted
• Current BLS certification required
• CHPN certification preferred
• Valid driver's license and reliable personal transportation with insurance required
• Minimum 1 year of Home Health experience required (no exceptions), with at least 2 years of overall clinical RN experience
• HCHB (Homecare Homebase) training/experience required — candidates without it must complete training and provide certificate of completion prior to start
• OASIS experience required
• Will not accept any hits on license
• Date of birth required for consideration
• RTO requests must be submitted at time of initial consideration — no RTO accepted after submission

• Responsible for comprehensive patient assessments, skilled nursing care delivery, and collaboration with certifying providers and interdisciplinary team members to develop and oversee individualized plans of care
• If a patient cancels or reschedules, the traveler must proactively contact the scheduler to add a new patient in order to meet the required daily and weekly point minimums
• Non-smokers preferred — the patient population includes individuals with COPD who are sensitive to cigarette smoke
• Scrubs in any color are acceptable
• Candidates who do not have HCHB training must complete it at their own cost ($250 upfront) before starting — reimbursement may be available; contact your recruiter for details
