Position: Registered Nurse (RN) - Home Health

Pay Range: $50.00–$65.00/hr
Status: Full-Time
Coverage Area: Coachella Valley

What You’ll Do

The Registered Nurse (RN) plans, organizes, and coordinates home health services while delivering high‑quality, patient‑centered nursing care. The RN uses strong clinical skills and community‑based nursing experience to support individuals and families in their homes, ensuring safe, effective, and compassionate care.

  • Develop and initiate the patient’s individualized plan of care and lead ongoing, interdisciplinary assessments in collaboration with the patient, their representative, and caregivers
  • Implement appropriate preventative, therapeutic, and rehabilitative nursing interventions as ordered by the physician and outlined in the plan of care
  • Provide thorough patient and caregiver education, including disease management, medication management, safety, and home‑based care techniques
  • Identify discharge planning needs early in the care process and ensure all necessary steps and education are completed prior to discharge
  • Coordinate delivery of medical equipment, supplies, and additional services to ensure patients have what they need for safe, effective care at home
  • Supervise Licensed Practical/Vocational Nurses (LPNs/LVNs) and Home Health Aides (HHAs), ensuring care is delivered in accordance with regulatory and organizational standards

Requirements

  • Valid CA Registered Nurse License
  • 2+ years of nursing experience, public health nursing or home health experience preferred
  • Maintains a current CPR certification
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
