Key Responsibilities

• Conduct in-depth assessments during sessions to identify and address the specific needs of each patient, ensuring individualized care plans are developed and maintained.
• Collaborate closely with a multidisciplinary team, including supervisors and support staff, to communicate patient progress and adjustments to treatment plans effectively.
• Schedule and facilitate home visits and therapy appointments, organizing your calendar to maximize efficiency and client support.
• Review and analyze case notes and other relevant patient documentation to inform treatment strategies and ensure compliance with legal and ethical guidelines.
• Engage in regular Interdisciplinary Team Meetings to discuss patient outcomes and strategies for enhancing service delivery.
• Prepare and maintain meticulous records that serve as both a therapeutic tool and a means of tracking progress over time, available for review by relevant stakeholders.
• Act as a liaison to the wider mental health community, fostering relationships that support the goals of both our clients and the organization.
• Conduct training sessions or workshops to ensure that staff are well-prepared to implement treatment plans effectively and adhere to the required documentation processes.

Patient Population / Client Focus

You will work with a diverse clientele, including adults, children, and families, each facing unique challenges that require tailored therapeutic interventions. Your role is crucial in providing empathetic support and strategies to help clients navigate mental health disorders, relationship issues, trauma, and substance abuse challenges.

Qualifications

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.
• Must possess an active and unrestricted LPCC license in California.
• A minimum of two years of clinical experience providing counseling in a healthcare setting is essential.
• A solid grounding in ethical and legal standards pertaining to mental health practice.
• Demonstrated ability to build rapport and trust with clients from various backgrounds, utilizing your interpersonal skills.
• Effective communication skills, both oral and written, are crucial for seamless collaboration and documentation.
• Proficiency in electronic medical records (EMR) systems and relevant mental health software to facilitate patient management and record keeping.

Preferred Experience

• Specialization in areas such as family therapy, couples therapy, trauma-informed care, or substance use treatment is strongly preferred.
• Experience utilizing evidence-based therapies and practices will be viewed favorably.
