Job description

Department: PHIL-HH Home Health Care

Schedule: Full Time, Days, 8am-4:30p

Location: Prime Healthcare Home Health Care

20000 Governors Drive Olympia Fields, IL 60461


Performs comprehensive assessments, evaluations and re-evaluations on patients to establish plan of care and goals. Performs patient treatment according to Physical Therapy procedures of modalities, therapeutic exercise, gait training and other accepted methods. Communicates pertinent information verbally or written to physician, other staff members, patients/caregivers. Supervises and directs Home Health Aide’s care in relationship to the patient’s rehab program. Demonstrates active participation in home care department functions.


EDUCATION, EXPERIENCE, TRAINING

1.    Bachelor, Masters, or Doctorate Degree in Physical Therapy from an accredited program.

2.    One year recent clinical experience.   Home Care experience preferred.

3.    Current license as a Physical Therapist. 

4.    Current BLS certification. 

5.    Current driver’s license, car insurance, car registration, and reliable automobile.
