Job description

Registered Nurse (RN) Opportunity

We are seeking an experienced and dependable Registered Nurse (RN) to provide field-based home health services across the Corpus Christi region. This role focuses on delivering high-quality patient care in a home setting, completing OASIS documentation, and managing a caseload independently. The ideal candidate has strong home health experience, is proficient in HomeCare HomeBase, and is comfortable working autonomously in the field.

Location: Corpus Christi, Portland, Taft, Ingleside, Aransas Pass, Rockport, Sinton, Odem, Beeville, Mathis, Robstown

Schedule: Day shift, typically Monday through Friday

Weekly Hour Guarantee: 40 hours

On-call/Weekend Requirement: Preferred 1 week every 6 weeks

Pay Rate: $51.00 to $53.00 per hour

Mileage Reimbursement: Yes, between patient visits from first patient to last patient

Key Responsibilities:

  • Provide skilled nursing care to patients in their homes
  • Conduct comprehensive patient assessments and manage ongoing care
  • Complete accurate and timely OASIS documentation
  • Utilize HomeCare HomeBase for documentation and care coordination
  • Develop and implement individualized care plans
  • Monitor patient progress and communicate changes to physicians and care teams
  • Educate patients and families on disease processes, medications, and care plans
  • Maintain compliance with agency policies and regulatory requirements
  • Participate in on-call rotation as assigned

Required Qualifications:

  • Active Texas Registered Nurse (RN) license
  • Previous home health field RN experience
  • Strong knowledge of OASIS documentation
  • Ability to manage a caseload independently
  • Strong clinical assessment and organizational skills
