An Extended Family Home (Shared Living Provider) opens their home for an individual with a disability. This position requires an open bedroom and the ability to help care for an individual with a disability. The goal of this service is to encourage and teach skill development so the individual can be as independent as possible. This client's needs are high-risk behavior, high risk with elopement and self-harm; therefore, restrictions and safeguards are in place accordingly.
This subcontracting position is a work-from-home role, where you provide care and support to a person in your own home, as well as offer transportation as needed. This position requires a desire to make a difference in an individual's life. Prime Home is here to assist and support you in the process and follow all State Regulation. SLPs are paid anywhere between $36,000-$105,000 per year, depending on the individual's funding received from the state. This is Tax-Free Money, as it is a protected income by law. Potential management of behavioral issues such as aggression, property destruction, etc.
The application and approval process is comprehensive and depends on several factors, including:
- Accuracy and completeness of application materials
- Completion of both phone and in-person interviews
- In-home walkthrough where services will be provided
- Required background checks (initial and annual) for:
- Provider
- Alternate caregivers
- All non-provider household members age 13 and older
Background checks include:- Criminal history
- APS/CPS registries
- Sex offender registries
- Completion of all required training (provided by Prime Home)
- Proof of:
- Valid auto insurance
- Homeowners or renters insurance
