What Youโ€™ll Do:

Perform in-home physical therapy evaluations and develop personalized care plans.

Deliver skilled therapy services focused on improving strength, balance, gait, and functional mobility.

Educate patients and family members on home exercise programs and safe mobility practices.

Collaborate with our interdisciplinary BrightStar team including nurses, OTs, HHAs, and case managers.

Document patient progress and visit notes using our mobile-friendly EMR system.

Ensure patient safety and quality outcomes aligned with BrightStarโ€™s core standards and compliance protocols.

Requirements:

Active and unrestricted Massachusetts Physical Therapist license

Graduate of an accredited PT program (DPT or MPT)

Minimum 1 year of PT experience preferred (new grads welcome with supervision)

Reliable vehicle and valid driverโ€™s license

CPR certification

Ability to work independently and as part of a care team
