  • Deliver Skilled Nursing Care - Provide high-quality, comprehensive nursing care in the patient's home in accordance with the physician's orders and individualized plan of care.ย 
  • Collaborate on Plan of Care - Partner with the interdisciplinary care team to develop, implement, and update personalized care plans that address each patient's unique needs.ย 
  • Manage Treatments and Interventions - Administer nursing treatments and manage clinical activities to support optimal health outcomes for each patient.ย 
  • Monitor and Evaluate Progress - Continuously assess patient progress and the effectiveness of services; communicate findings with the care team, patient, family, physician, and case manager.ย

Requirements
