Sage Group HealthCare Staffing is seeking Home Health Registered Nurse for a 13-week travel / contract position in Sheboygan, WI. Accepting both local and traveler candidates.
Position Details:
Position Name: Registered Nurse Home Health
Location: Sheboygan, Wisconsin
Specialty: Home Health

Contract Length: 13 weeks-Accepting Local and Travel Candidates
Pay Rate: Range $2000 to $2080 Weekly Gross pay based on a 40 hour work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments
Unit: Home Health
Shifts available: Days
Shift timings: 8 Hours; Days, 8a - 5p
Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri. Weekly productivity expectations - 30 points
Position Requirements:
  • Registered Nurse: 2 years - Required
  • Recent/Current Home Health Nursingexperience: 1 year within the last 3 years- - Required
  • OASIS experience Required
  • Home Care Home Base (HCHB)charting experience -Required
  • Must have valid Driver's Licenseand Personal Auto Insurance
  • Must be Covid Vaccinated orprovide medical or religious exemption
Education, License and Certification Requirements:
  • ASN or an equivalent - Required
  • BSN or an equivalent - Highlypreferred
  • RN License for State applying or Compact if accepted in that state - Required
  • BLS Certification from AHA orAmerican Red Cross - Required
Job Description:
  • Provide and coordinate care forassigned patients in comfort of their homes.
  • Develop and implement nursingcare plans including Start of Care and admissions as assigned.
  • Support patient recovery andability to stay at home - enabling them to spend quality time with theirloved ones and doing the activities they enjoy
  • Empower patients and families toreach healthcare goals by educating them on disease management
  • Case management and provideclinical oversight to interdisciplinary
  • Provide nursing care and serviceswithin the standards and scope of nursing practice as defined within thestate Nurse Practice Act regulations, policies, procedures, and protocols,as well as facility and corporate policies and procedures.
  • Ensure physicians' orders arelegible if taken verbally and transcribed appropriately per policy andprocedure.
  • Ensure patients' status isassessed on an ongoing basis and pertinent information is gathered anddocumented.
  • Notify physicians of significantchanges in status
  • Document and complete chartingfor all patient visits within 24 hours.
  • Territories to cover: All of Sheboygan county and northern Ozaukee county with a 30 mile radius of Sheboygan primarily.
