PRN Healthcare is seeking a travel nurse RN Home Health for a travel nursing job in Waukesha, Wisconsin.

**Home Health and Hospice Required - admission, death visit, or symptom management in hospice

Contract 13 week
Start: 8/10
Guaranteed Hours Per Week: 40
Primary Location: ProHealth Home Health, Waukesha, WI
Shift Requirements: 10 hr Day - SAT SUN MON TUES
Travel Requirements: 7-7.5 count in a 10 hr day, 50-100 miles
On Call/Call Back Requirements: none
Required Certifications to be Attached upon Submittal: BLS (American Heart), WI RN License, Valid Drivers License
Minimum Experience: 1-2 years current Home Health and Hospice Experience REQUIRED; Recent Oasis experience REQUIRED
Charting Experience Required: Epic Required within 12 months
Unit Information (Bed size, Case type, Patient ratio): 1:1 patient ratio; adults 18yrs and older, chronic disease management including CHF, COPD, Diabetes, Cancer, CVA, wound care, IV administration
Orientation will be during the day shift
Hard worker, Time management, Resilient and Manages Change well, Social candidates
Scrub Colors: plain colored scrubs, well maintained

**Needs to be able to adapt and be flexible, adjust to the ever-changing schedule that is home health and hospice, timely documentation, good solid skill set and assessment abilities, knowledgeable about home health OASIS and hospice. Able to hit the ground running

** Homecare experience a must, need to inquire about allergies as home environments are uncontrolled and may have pets and/or smoke, need to be able to physically tolerate driving throughout the day and up/down stairs and possible uneven landscapes for home entry, visits need to occur despite weather conditions

Covid-19 Vaccine Required unless Medical or Religious Exemption applies

GSA hours are not guaranteed the week of Orientation. Traveler is guaranteed set hours per week, except during holidays where unit is closed
