Job description

RN - Case Manager

Recent home health and OASIS experience required. Admission, death visit, or symptom management in hospice is a plus. Case Manager RN - Day shifts - ProHealth Care Home Care. Contract length: 13 weeks. Start date: 1/26. Guaranteed hours per week: 40 hours per week. Primary location: ProHealth Home Health, Waukesha, WI. Shift requirements: Mon-Fri 8hr days; 8-4:30pm. Travel requirements: 6 patient visits per day; approximately spending 1 hour per patient including drive time, patient visit, and charting. 6 county service area; up to 250 miles per week; average 50 miles per day. On call/call back requirements: none. Required certifications to be attached upon submittal: AHA BLS, active WI RN or compact license; valid driver’s license; OASIS cert a plus. Minimum experience: current home health experience required; OASIS experience required. Charting experience required: Epic within the past year. Unit information: 1:1 patient ratio; adults 18 years and older; chronic disease management including CHF, COPD, diabetes, cancer, CVA, wound care, IV administration. Would carry a caseload of patients and perform all OASIS time points. Resilient, manages change well, and has good time management skills. Covid-19 vaccine required unless medical or religious exemption applies.
