Job description

Position SummaryThe Nurse-Case Manager provides direct clinical coordination and documentation for assigned patients. Responsibilities include completing required nursing assessments, managing provider orders and medications, reviewing laboratory results, participating in interdisciplinary teams, and ensuring communication across care settings. This position includes an on call component.Clinical Assessments and DocumentationComplete nursing assessments & reassessments.Complete required transfer documentationDocument all assessments, care activities, provider communications, and clinical updates in the patients record within required timeframes.Medical ManagementReview laboratory results and report abnormal values to providers.Receive, clarify, and implement medication and treatment orders from providers.Communicate medication orders to pharmacies and follow up on incomplete or missing orders/medications.Reconcile Medication Administration Records to ensure accuracyPrepare and review weekly pillbox setups with patients, administer injections for internal providersProvide education to patients regarding medications, dosing, purpose, and potential side effects.Care CoordinationParticipate in interdisciplinary treatment team meetings with psychiatry, social work, district forensic coordinators, probation officers, home managers, dietitians, and other stakeholders.Provide medical updates and coordinate care with internal and external providers.Ensure required clinical information is shared to support continuity of care.On Call ResponsibilitiesRespond to after-hours calls from direct care staff regarding clinical concerns.Provide clinical guidance by phone and determine when an in person response is required.Respond to the facility after hours when necessary for medical emergencies, including medication access issues or acute patient needs.QualificationsLicensed Nurse, LPN or RN, with an unencumbered license.Must pass required criminal background check and drug screening.Experience preferred but not required.NEW NURSES WELCOMEProfessional ExpectationsReliable attendance and punctuality.Professional attire and grooming appropriate for a clinical setting.Strong verbal and written communication skills.Basic to intermediate computer skills, including use of electronic health records, email, and standard office systems. Work hours Monday -Friday 8:00am - 5:00pm.Paid holidays and weekends off.
