Please note: All opportunities at 21HHS require being in the field visiting patients in their homes. Remote/work-from-home positions are not available unless specifically noted. Full time Certified Home Health Aide (CHHA) Territory: Alameda County: Dublin, Livermore, Pleasanton and surrounding cities* Under the direct supervision of an RN Case Manager, you will provide patient care according to the Plan of Care. What you'll do as a Certified Home Health Aide (HHA):
  • Provide personal care including, baths, back rubs, oral hygiene, shampoos, and changing bed linen
  • Assist patients with dressing and undressing
  • Provide skin care to prevent breakdown
  • Assist the patient with toileting activities
  • Keep patient's living area clean and orderly, as appropriate
  • Assist in feeding the patient, if necessary.
  • Perform range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
  • Assist patient in the self-administration of medication.
  • Meet safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).
  • Report on patient's condition and significant changes to the assigned nurse.

To be a success as a Certified Home Health Aide
  • Valid CHHA CA Certification
  • Current BLS/CPR Certification
  • Valid CA Driver's License

  • Prior to scheduling an in-person interview, all Candidates are required to complete a computer literacy and competency assessment. $55,001 - $90,000 a year Estimated Wage range Certified Home Health Aide (CHHA): 55,000.00 - 90,000.00 Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (Up to 4% match) * You will be assigned a primary location; however, you may occasionally be asked to provide coverage outside of that area. This may include supporting vacancies, leaves of absence, higher patient census, or other client care needs.
