Description
We have a great opportunity for a Home Health RN!
This is a full-time position; Monday - Friday 8:00am - 5:00pm. The service area includesGreensboro, High Point, Kernersville, Winston- Salem, Lexington; [Forsyth and Upper Davidson Counties]. CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
We are seeking a dynamic, quality-driven Registered Nurse (RN) Case Manager to pave the way for outstanding care delivery for our population of home health patients. The ideal candidate will have a passion for providing exceptional, compassionate care to our home health patients, ensuring high-quality outcomes and improving the overall patient experience.

Requirements
Qualifications:
CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
  • Education:
    • Current, active Registered Nurse (RN) license in North Carolina (required).
    • Bachelor's degree in Nursing (BSN) preferred.
  • Experience:
    • Minimum of 2 years of experience in nursing, with at least 1 year in a home health or community health setting preferred.
    • Experience in leading or coordinating patient care is strongly preferred.
  • Skills and Abilities:
    • Strong clinical assessment and critical thinking skills.
    • Excellent leadership, communication, and interpersonal skills.
    • Ability to work independently while managing a caseload of home health patients.
    • Knowledge of home health care regulations, including Medicare and Medicaid guidelines.
    • Proficiency with electronic health records (EHR) and other healthcare software.
    • Strong organizational and time management skills, with the ability to prioritize patient needs.
    • Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
