Job description


Position: Registered Nurse, Case Manager

Pay: $2,812/Week

Hours: 40

Shift: Day Shift

Location: Dedham , MA

Is the hourly schedule 8-5PM? 40 Hours/Week Yes, can be less if needed.

Additional Info: 

Will they be working out of the Roslindale location and is there floating to the Dedham location? They will be working OUT of Dedham location, #29 Newbridge Way, but will be covering Brookline, Boston area. Hopefully you are aware (and if you are not hope this infio helps), that this is a Routine Hospice at Home position.  This person will be mostly making visits in patient's homes to provide hospice care.

How many Cases/Day should they expect? 3-5

Are they using Meditech EMR? Using Meditech at Home EMR


The RN Case Manager is responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with medical regime.

Minimum Requirements:

  • Current RN licensure in state practicing
  • At least one year of Case Management experience preferred
  • Current CPR if applicable
  • TB questionnaire, PPD or chest x-ray if applicable 
  • Current Health certificate (per contract or state regulation) 
  • Must meet all federal, state and local requirements 
  • Must be at least 18 years of age
