Position Responsibilities

The After-Hours RN provides clinical support outside of traditional business hours and helps ensure continuity of care while reducing unnecessary hospitalizations.

Responsibilities include:

  • Complete skilled nursing visits during evenings and weekends
  • Perform:
  • Start of Care (SOC) assessments 
  • Resumption of Care visits 
  • PRN visits 
  • Urgent nursing visits beginning at 4:00 PM 

Coordinate care with:

  • Physicians 
  • Case managers 
  • Interdisciplinary care teams 

Provide skilled clinical care including:

  • Comprehensive patient assessments 
  • Medication reconciliation 
  • Wound care 
  • Disease management education 
  • Catheter care 
  • IV starts

Support urology-related patient needs including:

  • Foley catheter management 
  • Nephrostomy care 
  • Urinary retention monitoring 
  • Post-surgical urology follow-up

Schedule Structure

Monday – Friday 
4:00 PM – 10:00 PM
