Job description

Job Title: Staff Chaplain
Department: Pastoral Care
Location: 800 W. Miller St., Newark, NY

or 330 Monroe Avenue, Rochester, NY
Hours Per Week: 32
Schedule:4 Six-Hour Shifts: Monday through Friday

Summary
The chaplain is responsible for assessment of and delivery of spiritual care to Lifetime Care patients, families, and as appropriate, staff. This often involves the education and support of the Interdisciplinary Team members, patient's own spiritual advisors and clergy in the community in dealing with the spiritual needs of the terminally ill and their families.

Essential Responsibilities/Accountabilities
Participates in timely assessment, delivery, and/or referral of appropriate spiritual care to patients, families, and staff within agency service area, as appropriate. Develops a pastoral care plan and interventions, which may include direct spiritual care, as well as providing for visits and/or rituals by patient/family's own clergy, if requested.
Supports the Interdisciplinary Group (IDG) care of patients with verbal communication and written documentation, as well as attendance at IDG and spiritual care department meetings and training as required.
Participates in cooperation with the bereavement department in assessing and providing for the ongoing spiritual needs of families.
Provides collaborative patient and family spiritual care by developing and maintaining relationships with community clergy through referrals and education about the end-of-life process and needs.
Maintains an ongoing network within the community in an effort to educate and recruit qualified volunteer chaplains to support the pastoral care department.
Consistently demonstrates high standards of integrity by supporting the Lifetime Healthcare Companies' mission and values and adhering to the Corporate Code of Conduct.
Maintains high regard for member privacy in accordance with the corporate privacy policies and procedures.
Regular and reliable attendance is expected and required.
Performs other functions as assigned by management.


Minimum Qualifications
Master of Divinity/Doctoral degree from an Association of Theological Schools accredited seminary. In lieu of degree, a minimum of five years of pastoral counseling/pastoral care practitioner experience. Ordained and endorsed minister, priest, rabbi or member of a religious order preferred.
Health care chaplaincy experience required. Palliative/Hospice experience preferred.
Ability to work comfortably with a variety of spiritual/religious practices, with a sensitivity to the specific needs of patients who are terminally ill and their family members.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
