Job description

A-Line Staffing is now hiring a Hybrid Case Manager (RN, LCSW, or LCPC). This will be full time.

  • Work Setting/Location: Candidates can live anywhere in IL. This is a hybrid role; will be mostly WFH with some member visits in the area they live as needed.
  • Schedule: Mondayโ€“Friday | 8:00 AM โ€“ 5:00 PM CST
  • Pay Rate - $35.87 an hour
  • Benefits โ€“ medical, dental, vision, life insurance active after 90 days

Key Responsibilities

  • Conduct comprehensive assessments using clinical tools and data review
  • Evaluate member eligibility, needs, and available benefits/resources
  • Develop and implement individualized care management plans
  • Coordinate care with providers, caregivers, and community organizations
  • Support members with medical, behavioral, and social determinant needs
  • Apply clinical guidelines, regulatory requirements, and company policies appropriately
  • Ensure compliance with case management and quality management standards
  • Advocate for members to promote positive health outcomes and cost-effective care

Required Qualifications

  • Active, unrestricted Illinois license in one of the following: RN, LCSW, LCPC
  • 2โ€“3 years of: Care Management, Discharge Planning, Home Health Care Coordination
  • Experience with IL Waiver Services
  • Experienced with Medicare/Medicaid Populations
  • Experienced managing high volume caseloads
  • Case Management Certification (CCM) preferred
