Job description

Position: Case Manager (CDCA)

Job Summary: Provide support services and foster community by assessing the needs of their clients and developing and implementing a comprehensive plan to address those needs. This position reports to a Case Management Supervisor.  

Duties and Responsibilities: 

  • Coordinates and provides safe, timely, effective, efficient, equitable, culturally sensitive, and client- centered care. 
  • For SUD Clients: Handles case assignments, assesses barriers to recovery in the various life domains, may draft SUD case management-specific service plan, review progress in case management goals. 
  • For MH Clients: Handles case assignments, assesses barriers to recovery in the various life domains, coordinates mental health treatment planning including goals for CPST services, provides input, and works collaboratively with the clinical counselor, psychiatrist, and/or nurse practitioner in the development of the treatment plan. 
  • Is familiar with the diagnostic assessment completed for the clients served, which includes a review of diagnosis, identification of current symptoms, risk, functioning, identified needs in the client's life domains shown as Z-Codes to determine eligibility for Case Management or CPST services and the level of care needed. 
  • Works collaboratively following up with observations with the clinical counselor, psychiatrist, and/or nurse practitioner in the development of the treatment plan. 
  • Adheres to Lighthouse's protocol for appropriate "care management" activities as part of the continuum of services to meet each client's defined needs and as defined in the treatment plan. 
  • Provides follow-up and appropriate referrals to community resources are essential. Collaboration and coordination of services to ensure continuity of care and efficiency of services across systems is vital to early recovery. 
  • Provides linkage and assistance in accessing inpatient or more restrictive treatment, as medically necessary. 
  • Stays abreast of best practices in modalities of intervention to maintain high-quality care management that is medically necessary to client specific treatment plan. 
  • Evaluates daily living functioning and assist clients in creating goals to improve daily living. 
  • Completes billing and documentation accurately in a timely manner. 
  • Sees clients weekly or as clinically appropriate according to the client's level of care. 
  • Ensures all community activities comply with HIPAA, 42 CFR Part 2, and Mental Health laws and regulations on confidentiality of PHI. 
  • Keeps abreast of community resources and makes needed referrals to meet client needs on a timely basis. 
  • Coordinates and collaborates with other community agencies regarding client's treatment to promote positive treatment outcomes. Participate in community collaboration meetings. 
  • Advocates and raises awareness on behalf of clients for community and local services to meet client needs.

Required Qualifications: 

  • CDCA certification
  • Current State of Ohio Driver's License and active insurance
  • Must pass BCI check, all Corporate Compliance checks, and employment drug screen  

Desired Qualifications:

  • Knowledge of Job and Family Services procedures
  • Bachelor's Degree in Social Work, Psychology, Counseling, or a directly related human service field. 
  • 2 years relevant experience in case management, social services, or a related field 

Schedule: Monday - Friday 8:30am - 5:00pm

Compensation range: $20.19 - $26.60
