Exciting opportunity with a growing agency! Chestnut is still expanding its service offerings after almost 50 years of providing care. Our Family Health Center promises to be a busy place given new partnerships and collaborations. Be part of the team that is the region’s leading provider of integrated primary and behavioral health care.  Position is full-time and is in Bloomington, IL

Health Center hours are Monday – Friday 8:00 a.m. to 5:00 p.m. and Thursday 8:00 am – 7:00 pm.  FLEXIBLE SCHEDULING OPTIONS! Sign-on and retention bonuses available! Tuition reimbursement and loan repayment opportunities are also available.


Provide high-quality, patient-centered, culturally sensitive mental and primary health care services. Assess patient needs and create care treatment plans. Manage patient care. Staff cases as appropriate. Provide direct medical prescriber services. Help develop programs.

Promote the recovery model of treatment including trauma-informed care, hope, respect, empowerment, health/wellness, and spirituality/connectedness.


RN with Master’s degree in Nursing. Licensed in Illinois as Nurse Practitioner or other Advanced Practice Registered Nurse. Specialization in mental health preferred; specialization in family practice with experience in mental health or substance use setting acceptable.
