Job description

We are looking for a skilled and compassionate Registered Nurse (RN) to join our Home Health & Hospice team. As a field RN, you will play a vital role in delivering high-quality, patient-centered care to individuals in their homes—promoting comfort, dignity, and independence.


Key Responsibilities:

  • Conduct comprehensive patient assessments and develop individualized care plans
  • Provide skilled nursing services including wound care, IV therapy, symptom management, and medication administration
  • Monitor and document changes in patient condition and coordinate care with physicians and interdisciplinary team members
  • Support patients and families with education, resources, and emotional guidance throughout the care process
  • Participate in the on-call rotation, including evenings, weekends, and holidays as needed
  • Ensure timely, accurate documentation in compliance with agency policies and regulatory standards


Qualifications:

  • Current RN license in the State of Texas
  • Current CPR certification (BLS required)
  • Minimum of 1 year of clinical nursing experience; home health or hospice experience strongly preferred
  • Bilingual (English/Spanish) preferred
  • Strong clinical judgment, time management, and communication skills
  • Reliable transportation, valid driver’s license, and automobile insurance (required for field-based visits)
