Job Title: Weekend RN Admissions Nurse | Home Health | $200 Per Start of Care Visit Company: Home Health Care, Inc. You Know What A Good Admission Looks Like A great Start of Care nurse sees more than what's written on the referral. You notice the medication discrepancy. You recognize the fall risk. You identify when the caregiver needs more education. You ask the question that uncovers an important change in condition. And you understand that accurate OASIS documentation and a strong initial plan of care can influence everything that happens throughout the patient's home health episode. That's the expertise we're looking for. What You'll Do Complete weekend Start of Care admissions for new home health patients. Perform comprehensive skilled nursing assessments. Complete OASIS and admission documentation accurately and on time. Review medications, diagnoses, symptoms, safety concerns, and caregiver needs. Develop and initiate individualized plans of care. Provide patient and caregiver education. Identify additional clinical and therapy needs. Coordinate care with physicians, nurses, therapists, and other interdisciplinary team members. Communicate significant clinical findings and changes appropriately. Who We're Looking For This is not intended to be an entry-level home health opportunity. We're looking for an RN who can confidently manage the complexity of an admission in the field. Required Qualifications Active Minnesota Registered Nurse license. Current CPR certification. Previous home health nursing experience. Strong OASIS knowledge. Start of Care or admission experience strongly preferred. Strong assessment and clinical decision-making abilities. Excellent documentation and communication skills. Valid driver's license, active insurance, and reliable transportation. Already Working Full-Time? That's Okay.
