Job Title: Weekend RN Admissions Nurse | Home Health | $200 Per Start of Care Visit Company: Home Health Care, Inc. Location: Twin Cities Metro Area Schedule: Weekends Only Compensation: $200 per Start of Care Visit + Mileage Reimbursement Skip The 12-Hour Shift. Use Your Weekend Differently. If you're an experienced home health RN who knows Start of Care admissions and OASIS, Home Health Care, Inc. offers a different way to put that expertise to work. Instead of committing to another hospital shift, managing an ongoing caseload, or adding weekday hours to an already busy schedule, you can focus specifically on weekend Start of Care admissions. You'll be compensated $200 per SOC visit plus mileage reimbursement while using the assessment and clinical decision-making skills you've already worked hard to develop. That's the expertise we're looking for. What You'll Do Complete weekend Start of Care admissions for new home health patients. Perform comprehensive skilled nursing assessments. Complete OASIS and admission documentation accurately and on time. Review medications, diagnoses, symptoms, safety concerns, and caregiver needs. Develop and initiate individualized plans of care. Provide patient and caregiver education. Identify additional clinical and therapy needs. Coordinate care with physicians, nurses, therapists, and other interdisciplinary team members. Communicate significant clinical findings and changes appropriately. Who We're Looking For This is not intended to be an entry-level home health opportunity. We're looking for an RN who can confidently manage the complexity of an admission in the field. Required Qualifications Active Minnesota Registered Nurse license. Current CPR certification. Previous home health nursing experience. Strong OASIS knowledge. Start of Care or admission experience strongly preferred. Strong assessment and clinical decision-making abilities. Excellent documentation and communication skills. Valid driver's license, active insurance, and reliable transportation. Already Working Full-Time? That's Okay.
