This role is conducted entirely through telehealth services, offering counselors the flexibility to deliver care from the comfort of their own homes or any suitable remote location. Your primary responsibilities will include:

• Conducting patient intakes, including comprehensive diagnostic and functional assessments, and making informed recommendations to the primary care team for treatment goals.
• Providing brief, focused interventions to patients requiring mental health services, ensuring they receive quality care tailored to their needs.
• Collaborating with primary care providers to identify patients' mental health needs and recommending referrals to specialty services when appropriate.
• Monitoring patient progress and making necessary adjustments to treatment plans based on follow-up evaluations.
• Developing relapse prevention plans that support ongoing stability and wellness for patients at risk of deterioration.
• Documenting patient interactions and progress comprehensively within the treatment chart, ensuring all information is up-to-date and accessible for team collaboration.
• Assisting patients in adhering to prescribed medical treatments and developing self-management skills for long-term health improvement.
• Participating in team meetings to consult on behavioral health services and contribute to the clinic's program evaluation and improvement efforts.

As a counselor at Crystal Lake Families & Kids Services, you will serve a diverse range of clients, including children, families, and adults from varied backgrounds. Many of the patients present with complex mental health challenges that intersect with medical diagnoses, requiring a nuanced understanding of both psychological and physical health. Your role will focus on equipping clients with the knowledge and skills necessary to manage their conditions and enhance their quality of life.

The ideal candidate will possess:

• A Master's Degree in Clinical Counseling or a closely related field.
• Current licensure as a Licensed Clinical Professional Counselor (LCPC) in Maryland.
• Extensive knowledge of mental health assessment techniques, treatment planning, and psychotherapeutic modalities, including family and group therapy.
• Strong understanding of the personal and social implications of mental and physical health issues.
• Proficiency in documentation practices and the management of clinical records.
• Ability to establish and maintain trusting relationships with patients, families, and the multidisciplinary team.
• Excellent communication skills, with the ability to articulate therapeutic concepts and goals clearly.

While not required, bilingual proficiency in English and Spanish is preferred, as this will help in serving a broader client base effectively.
