• Conduct thorough patient intakes focused on diagnostic and functional evaluations, formulating treatment goals in collaboration with the primary care provider.
• Provide brief, focused interventions to patients experiencing mental health challenges, utilizing evidence-based therapeutic techniques.
• Advise the primary care team on which patients might benefit from specialized mental health facilities or services, ensuring seamless transitions of care.
• Develop and initiate follow-up plans for continuous assessment of patient progress, adjusting treatment strategies based on feedback and observed outcomes.
• Create relapse prevention plans aimed at empowering patients to maintain stability and avoid setbacks in their mental health journey.
• Monitor and coordinate holistic health services related to behavioral care, linking patients to additional treatment providers upon consent.
• Document patient progress meticulously, maintaining accurate clinical records that reflect diagnostic information and treatment plans.
• Consult with interdisciplinary team members to enhance the delivery of care and continuously evaluate service outcomes.
• Empower patients through education and self-management strategies, equipping them with tools to actively participate in their recovery processes.
• Adhere strictly to the Code of Ethical Conduct for Clinical Professionals while executing all responsibilities.

The LPC will serve a diverse client population, ranging from children to adults, many of whom face various mental health challenges intensified by social determinants of health. The ideal candidate will demonstrate a strong commitment to understanding and addressing the unique needs of our patients, fostering a therapeutic rapport that encourages open communication and trust.

• A Master's Degree in Clinical Counseling is essential.
• Current LPC (Licensed Professional Counselor) License in good standing.
• Comprehensive knowledge of mental health assessments, treatment planning, and therapeutic methodologies, including family therapy and behavioral therapy.
• Strong understanding of DSM-IV diagnoses and psychodiagnostics.
• Ability to collaborate effectively within an interdisciplinary team setting and communicate treatment plans and progress transparently.
• Proficient in maintaining clinical documentation in compliance with all applicable regulatory standards.

• Bi-lingual English-Spanish communication skills are highly desirable but not mandatory.
• Experience working within a Federally Qualified Health Center (FQHC) is beneficial.
• Familiarity with community resources and pathways to ensure comprehensive patient care.

The salary range for this position is competitive, starting at USD 87,000 and reaching up to USD 105,000 annually based on experience and qualifications.
