Position Summary

The Care Progression Supervisor is responsible for providing day-to-day operational oversight and leadership for Case Management and Social Work activities across inpatient units. This role supports the department's goals of timely and effective patient discharges, coordinating with multidisciplinary teams, ensuring appropriate staffing coverage, and fostering cross-unit collaboration. The Care Progression Supervisor plays an active and instrumental role in department onboarding for new hires, initial and ongoing education, and identification of departmental opportunities to drive facility length of stay management.

Key Responsibilities

  • Staffing & Scheduling
    • 40 Hours - M-F - in person, some weekend requirements possible

    • Oversees daily staffing assignments to ensure appropriate coverage across care progression roles.
      • Remains active team member and is expected to absorb a case load as necessitated by coverage demand.
    • Manages scheduling needs, including approval and tracking of PTO requests and shift adjustments.
  • Care Coordination & Patient Flow
    • Conducts daily long-stay patient reviews to assess discharge barriers and drive resolution.
    • Facilitates communication and collaboration among interdisciplinary teams to promote timely and effective discharges.
    • Leads or participates in site-level and unit-based huddles.
    • Contributes to patient round efforts at a site-level.
  • Discharge Planning Support
    • Provides coaching and instruction to staff on complex discharge planning cases.
    • Serves as a resource for clinical and non-clinical teams in resolving discharge delays.
  • Departmental Leadership & Development
    • Identifies trends, barriers, and opportunities to improve care progression workflows.
    • Participates in and/or leads quality improvement initiatives related to discharge planning and patient throughput.
    • Contributes to ongoing departmental education, including the development and delivery of training materials.
    • Participates in hiring decisions, discipline and evaluations.
  • Onboarding & Staff Education
    • Works in collaboration with Care Progression Manager for training and development of staff.
    • Coordinates onboarding activities for new hires, ensuring training is timely, effective, and aligned with departmental standards.
    • Supports continuing education efforts for the care progression team to enhance competencies and stay current with best practices.
    • Partners in responsibility for all resources and functions related to regulatory compliance adherence

Education:Bachelor's degree in Nursing or Master's in Social Work

Experience:Minimum of 3 years in case management, discharge planning, or utilization review. Supervisory or lead experience strongly preferred.

Licensure/Certification:Active RN, LMSW/LCSW, or related licensure required, depending on background. Certification in Case Management (CCM, ACMA) preferred
