Psychiatric Nurse Practitioner, (FQHC), Flexible Schedule - New York
A leading not-for-profit, Federally Qualified Community Health Center with locations in Manhattan, the Bronx and the Mid-Hudson Valley region of New York State is looking to add a Psychiatric Nurse Practitioner in Manhattan. The Nurse Practitioner provides diagnostic interviews, medication management and therapy to patients as well as consultation to physicians, social workers and other staff members. Conduct health assessment interviews. Perform complete physical assessment. Order and interpret diagnostic tests. Develop plan of care based on the diagnosis and individual client needs. Prescribe medications and treatments, as appropriate. Work collaboratively with the health care team and makes referrals to other disciplines, as indicated. Record, report, and interpret the clients responses to plan of care. Teaches, counsels and assists individuals and families to assume responsibility for prevention of illness and the promotion, maintenance and restoration of health. Collaborate with other members of the team in the provision of regular health screening and other health promotion measures. Identify available community resources helpful to clients, refers and assists clients in utilizing such resources Participate in CQI projects. Provide supervision and guidance to psychosocial staff members and students. Facilitate on-going chart reviews. Conduct educational sessions for psychosocial staff, outside agencies and primary care providers. Document all patient care in electronic medical record Monday - Friday Flexible Work Schedule. Choice of locations include Harlem, Brooklyn, Queens, Manhattan or The Bronx
