Job description

Enriches the lives of participants by planning, organizing, and executing a variety of engaging activities designed to promote social interaction, physical wellbeing, and continuous learning through implementation of  innovative programming for specified populations which may include the frail elderly, developmentally disabled, and/or chronically mentally ill.  Assesses the needs, interests, and capabilities of participants for utilization in development recreation plans.  Promotes and supports the objectives of Aging Services, specifically dementia and performs all defined services and other related duties in accordance with the mission of Aging Services.

Develops and implements a monthly calendar of activities including organizing community outings and special events for specified participant population.   
Plans and implements group and individual activities involving cognitive, physical, social, psychosocial, spiritual, and artistic needs and interests of participants.   
Completes evaluation and assessment of individuals interests, activity preferences, and assistance needs during recreational activities within 30 days of enrollment and annually.  
Completes daily documentation in accordance with Federal, State or local regulation and Aging Services policies.  
Maintains a clean and safe work environment including program areas being maintained and ready for use. 
Assists with training and indirect supervision of activities volunteers. 
Maintains a reference library of activity materials, entertainment, and community events.  
Participates in interdisciplinary team meeting.  
Provides personal care and services to assist participants with activities of daily living, including toileting, showers, eating assistance, grooming and oral care.  
Performs laundry, housekeeping, direct care, and other duties as assigned. 
May drive participants to and from the center.  


High School Diploma/GED, required

Bachelor's Degree, preferred 

1 year experience creating and implementing activities for elderly, developmentally disabled, and/or mentally ill, required 

CPR Certification, within 90 days of hire

Dependent Adult Abuse, within 90 days of hire
