Primary Registered Nurse - Meng Residence

The Primary Registered Nurse works collaboratively with a team of nurses to support case management at the Meng facility, including completing skilled nursing assessments for routine, comprehensive, and higher-acuity visits. This role requires flexibility, strong teamwork, and excellent communication skills. The Meng Residence is a 12-bed inpatient hospice facility in De Pere that provides 24/7 care in a peaceful, home-like environment for patients and families.


During times when census is higher, the nurse is cross-trained and may support the team by managing a small patient caseload, working as a float RN within the community, and providing care to patients in their homes or other facilities. This flexible approach helps ensure consistent, high-quality care across all settings as patient needs fluctuate.


Schedule: This is a full-time position - 36 hours per week, working 3x 12-hour shifts per week from 6:00 p.m. to 6:30 a.m., including an every-other-weekend rotation and working about three to four holidays per year.


Job Duties:

1. Provide clinical and supporting case management based on patientโ€™s diagnosis and end-of-life symptoms

2. Complete initial, comprehensive and updated assessments of patient and family needs

3. Develop, implement and review care plans for patient and family. Update care plan as the needs of the patient and family change

4. Prepare for and participate in interdisciplinary team meetings as scheduled

5. Communicate with all interdisciplinary team members

6. Report appropriate findings and communicate significant changes to attending physician and process physician/NP orders

7. Manage individual patient medication and Residence contingency medications

8. Administer and titrate medication according to physician/nurse practitioner orders

9. Delegate nursing duties to LPN and supervise care provided by CNA

10. Prioritize daily work and make adjustments while taking into account the needs of patients, Unity and other staff

11. Complete new patient admissions as needed

Minimum Requirements:

  1. Current unrestricted Registered Nurse licensure from the State of Wisconsin
  2. Minimum of 2 years RN experience required; equivalent nursing experience may be considered
  3. Valid Wisconsin Driverโ€™s License, reliable transportation, and proof of current automobile insurance coverage
