Your Responsibilities

As a Home Health RN Case Manager, your duties will include:

  • Conducting skilled nursing visits in patients' homes
  • Completing assessments, start-of-care visits, and developing care plans
  • Collaborating with the interdisciplinary care team
  • Overseeing LPNs and Home Health Aides
  • Documenting care using an iPad with AI assistance (OASIS experience is a plus)
  • Educating patients and their families to foster independence
  • Ensuring continuity, quality, and safety in patient care

What We Expect From You
  • Active, unrestricted Registered Nurse (RN) license
  • Valid driver’s license and reliable transportation
  • CPR certification
  • At least 1 year of RN experience (home health or acute care preferred)

Preferred Qualifications
  • Experience working with aging or adult patient populations
  • Highly organized with excellent time-management skills; capable of multitasking, prioritizing a busy caseload, and confidently using technology and electronic documentation systems
