Are you a compassionate and dedicated Physical Therapist (PT) looking for a flexible per diem opportunity? We are seeking an experienced Physical Therapist to join our healthcare team in Green Cove Springs, Florida and help patients improve mobility, regain independence, and achieve their highest level of function.

Core Responsibilities Patient Evaluation
  • Assess patients' functional status, strength, mobility, and physical limitations.
  • Develop individualized treatment plans designed to achieve optimal patient outcomes.
Therapeutic Interventions
  • Provide skilled physical therapy services and evidence-based treatment interventions.
  • Monitor patient progress and adjust treatment plans as needed to maximize recovery and independence.
Education
  • Educate patients, families, and caregivers on therapeutic exercises, adaptive techniques, injury prevention, and proper use of assistive devices.
  • Promote patient engagement in the rehabilitation process.
Documentation
  • Accurately document evaluations, treatment plans, progress notes, and patient outcomes.
  • Ensure compliance with facility policies and regulatory requirements.
Team Collaboration
  • Collaborate with physicians, nurses, case managers, and other healthcare professionals to coordinate care and support successful patient outcomes.
  • Assist with care transitions and discharge planning as appropriate.
Qualifications & Requirements

✅ Bachelor's or Master's Degree from an accredited Physical Therapy program

✅ Minimum of 1 year of Physical Therapy experience required, preferably in an acute care setting

✅ Active Florida Physical Therapist License in good standing

✅ Current Basic Life Support (BLS) certification required within 30 days of hire

✅ Strong assessment, treatment planning, and patient education skills

✅ Ability to stand, walk, bend, kneel, and safely lift or move patients and equipment weighing up to 50 pounds
