• Conduct comprehensive patient assessments to evaluate mobility, strength, and functional abilities, ensuring that each patient's unique needs are addressed.
• Develop individualized treatment plans in collaboration with physicians and other healthcare providers, focusing on evidence-based interventions and patient-centric goals.
• Deliver direct physical therapy services in the comfort of patients’ homes, utilizing specialized therapeutic techniques to enhance recovery and rehabilitation.
• Monitor patient progress and adapt treatment plans as necessary, documenting all interactions and outcomes in compliance with regulatory standards.
• Educate patients and their families on therapeutic exercises, mobility techniques, and the use of assistive devices to promote adherence to home exercise programs.
• Prioritize caseload management, ensuring timely and effective care delivery while coordinating with other healthcare professionals for integrated treatment.

The Physical Therapist at Flex Well Rehabilitation will serve a diverse patient population, ranging from pediatric to elderly individuals, many of whom are recovering from surgeries, experiencing chronic conditions, or rehabilitating from injuries. This role is vital in providing therapeutic support tailored to enhance physical functioning and quality of life for each patient, addressing unique challenges and promoting independence in everyday activities.

• A valid state license in Physical Therapy.
• A Doctor of Physical Therapy (DPT) degree from an accredited institution.
• Basic Cardiac Life Support (BCLS) certification, to be obtained within 30 days of employment.

• Experience in home health care or a similar setting is preferred but not required.
• Familiarity with electronic medical record (EMR) systems for efficient documentation and communication.
