Work Environment

This position offers a remote work setting, allowing for flexible employment while maintaining a strong connection to our interdisciplinary team. Regular virtual meetings and collaboration tools will be utilized to ensure effective communication within the team.

Key Responsibilities

• Conduct thorough assessments of patients’ mental health and psychosocial status, considering emotional, behavioral, and social functioning.
• Develop and implement individualized, evidence-based treatment plans that are culturally sensitive and aligned with the patients’ needs.
• Deliver psychotherapy focused on addressing mental health conditions such as anxiety, depression, and post-traumatic stress disorder (PTSD), specifically in the context of hospice care.
• Continuously monitor and evaluate patients' progress, making necessary adjustments to treatment plans based on patient feedback and response.
• Provide education to patients and their families about mental health issues, treatment options, and available community resources.
• Collaborate effectively with a multidisciplinary team, including psychiatrists, physicians, and nursing staff, to ensure comprehensive and coordinated care for each patient.
• Adhere strictly to ethical standards, including confidentiality, informed consent, and professional conduct.
• Utilize electronic health record (EHR) systems to manage patient documentation and care coordination in an efficient manner.

Patient Population / Client Focus

The LCSW will work primarily with patients receiving hospice care, many of whom may be experiencing significant mental health challenges associated with grief, loss, and the impact of terminal illness. Understanding the unique emotional turmoil faced by patients and their families is critical for developing effective therapeutic relationships and providing appropriate support during a sensitive time.

Qualifications

• Master's degree in Social Work from a regionally accredited institution.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in New Jersey.
• A minimum of 2-3 years of experience providing psychotherapy and mental health services, preferably within a healthcare or hospice setting.
• Exceptional communication and interpersonal skills to build rapport with patients and collaborate with a diverse healthcare team.
• Strong clinical acumen, including proficiency in assessments and treatment planning methodologies.
• Experience with electronic health records (EHR) and familiarity with other healthcare documentation systems is preferred.
• A deep understanding of mental health issues and the psychological challenges faced by patients in hospice care.
• Commitment to maintaining confidentiality and adhering to professional ethics in all aspects of practice.

Preferred Experience

• Experience in a hospice, palliative care, or end-of-life care setting is highly desirable.
• Familiarity with community resources available to support patients and families experiencing grief and loss.
• Proficiency in evidence-based therapeutic modalities and a commitment to ongoing professional development.

Compensation and Benefits
