Position Overview

Terros Therapy Center is seeking a dedicated and compassionate Licensed Independent Clinical Social Worker (LICSW) to join our team in Springfield, Massachusetts. Work Environment

This role is primarily remote, allowing you the flexibility to engage with clients and collaborate with colleagues from the comfort of your own home while still being an integral part of the hospice team. Key Responsibilities
  • Act as a vital liaison between clients, families, and outpatient providers to ensure a seamless continuum of care.
  • Engage with insurance companies for pre-certification and utilization management processes, ensuring clients receive the services they need in a timely manner.
  • Contribute to discharge and aftercare planning, ensuring that appropriate support systems are in place for clients post-discharge.
  • Provide therapeutic support during meal times, guiding clients in fostering a healthy relationship with food.
  • Offer safe, empathetic care that acknowledges and adapts to the unique intersecting identities of each client.
Patient Population / Client Focus

This position primarily supports older adults who are navigating complex emotional and social challenges related to end-of-life care. Qualifications
  • A Master’s degree in Social Work from an accredited institution is required.
  • A valid LICSW license in the state of Massachusetts is essential for this role.
