SALARY RANGE: 136,406.40 - 211,411.20 YEARLY
General Summary:
The Population Health Pharmacist is a licensed pharmacist that will provide support for clinical pharmacy services to participating health plans. Responsibilities will be focused on improving the health of Delaware residents by improving quality of pharmacy-related care (STAR measures) and cost containment strategies through provider collaboration with providers engaged in value-based arrangements at Bayhealth. The Population Health Pharmacist is responsible for providing clinical pharmacy support to Medicare and Commercial providers.
Responsibilities:
1. Support Bayhealth providers by analyzing reports and EMR data extraction to identify opportunities that will improve outcomes based on medication usage (utilizing STAR and HEDIS) and reduce pharmacy costs to health plan members.
2. Assess health plan members with pre-determined medical conditions to identify barriers to care, perform self-management education, recommend screening, perform MTM services, and develop and implement personalized health improvement plans and goals as well as provide support for the members to achieve and maintain these goals. The Pharmacist will use evidence-based guidelines and health strategies in collaboration with the health care team to achieve overall health goals of members
3. Collaborate with Bayhealth providers to implement clinical quality data initiatives as determined by Bayhealth and participating health plans. Must maintain a 5-STAR rating of 90 % or greater for members on an annual basis
4. Participate in various committees, meetings, and audits as determined by Bayhealth and the health plan. Develop and maintain policies and procedures that supports Bayhealth's initiatives and meets State and Federal legal requirements and standards.
5. Inform providers and health plan members of the health plan's formulary changes on an annual basis through educational programs.
6. Assists in the identification of health plan members needs and intervene as appropriate to prevent avoidable hospital admissions or readmissions. Document these interventions to assess success of program.
7. Act as a subject matter expert in pharmacy-related clinical measure for value-based care agreements.
8. All other duties as assigned, within the scope and range of job responsibilities.
Required Education, Credential(s) and Experience:
  • Education: Bachelor Degree Pharmacy
  • Credential(s): Licensed Pharmacist, Board of Pharmacy Specialties (BPS) Certified in a specialty, Board of Pharmacy Specialties (BPS) Certified in Pharmacotherapy, Ambulatory Care, Cardiology, Geriatrics, or related area.
  • Experience: Required: One year of hospital pharmacist experience as a clinical pharmacist or completion of ASHP accredited PGY-1 residency program.
    Successful completion of Bayhealth credentialing process if applicable.

Preferred Education, Credential(s) and Experience:
  • Education: Doctorate Degree Pharmacy
  • Credential(s): Board Certification with one year of clinical pharmacy experience or PharmD with Board Certification in Pharmacotherapy or advanced degree (ex. MPH or MBA)
  • Experience: Preferred: Two years of hospital pharmacist experience as a clinical pharmacist or completion of ASHP accredited PGY-2 residency ambulatory program.
