Nurse Practitioner Primary Care & Hormone TherapyPosition Overview

Agave Adult Medical Care is seeking a skilled and reliable Nurse Practitioner to provide in-clinic primary care and hormone therapy services for our adult and geriatric patient population. This role focuses on delivering high-quality, evidence-based care while maintaining strong patient relationships. You will manage chronic diseases, conduct routine primary care, and support patients through hormone management protocols.

You'll work closely with the owner and clinic staff to ensure smooth clinic flow and excellent patient experience. Please note, the opportunity to become part-owner/engage in a partnership is available.

Key ResponsibilitiesPrimary Care
  • Conduct routine physical exams and wellness visits.

  • Diagnose and manage chronic conditions (HTN, DM, COPD, CHF, thyroid disorders, etc.).

  • Develop and adjust treatment plans based on evidence and patient goals.

  • Order and interpret labs, imaging, and diagnostics.

  • Manage medication refills and ensure proper documentation.

  • Provide patient education, lifestyle counseling, and follow-up care.

Hormone Therapy
  • Evaluate patients for hormone deficiency or imbalance (women and men).

  • Review labs related to hormone health (testosterone, estrogen, progesterone, thyroid, cortisol, etc.).

  • Develop individualized hormone optimization plans following clinic protocols.

  • Prescribe hormone replacement therapies (HRT) and monitor treatment response.

  • Adjust dosages and follow up on side effects, safety labs, and treatment adherence.

  • Work in coordination with clinic leadership to refine hormone therapy workflows.

Operational & Compliance
  • Complete documentation in EMR in a timely and accurate manner.

  • Maintain compliance with Arizona state regulations and clinic protocols.
  • Support seamless patient flow and collaborate with clinic staff.

Qualifications
  • Active Arizona Nurse Practitioner (NP) license with prescriptive authority.

  • Board-certified in FNP, AGNP, or similar.

  • Experience in primary care (preferred).

  • Experience with hormone therapy, HRT, men's/women's health, or functional medicine

  • Strong communication skills and ability to build trust with adult/geriatric patients.

  • Comfortable working independently in a smaller clinic environment.

  • Professional, reliable, and patient-focused demeanor.
