The Finch seeks a compassionate, organized, and motivated Designated Coordinator to support a vulnerable adult with mental health needs in a structured, person-centered environment. This role is ideal for someone passionate about advocacy, relationship-building, and helping individuals maintain independence while ensuring their safety and overall well-being.

At The Finch, our client is often highly independent, but still require 24-hour staffing support due to vulnerabilities related to mental health, safety awareness, risk factors, and decision-making abilities. Our approach focuses on balancing independence with consistent support, guidance, and accountability.

Job Summary

As a Designated Coordinator House Manager, you will be responsible for overseeing the daily operations and providing comprehensive support to individuals with developmental disabilities in an Individualized Home Supports program setting.

You will ensure a safe, nurturing, and empowering environment for the resident(s), fostering their independence and promoting their overall well-being. The role requires strong leadership, excellent communication skills, and the ability to collaborate with various stakeholders.

Supervisory Responsibilities

Directly supervise and lead a team of support staff and direct care professionals.

Provide guidance, training, and performance feedback to ensure the team delivers high-quality care and support.

Responsibilities

Individualized Home Services Program Management:

  • Develop and implement individualized care plans for each resident in coordination with their support team and legal guardians.

  • Oversee the implementation of behavioral support strategies and ensure adherence to established protocols.

  • Manage the overall daily schedule and activities while ensuring a balance between individual preferences, community engagement, and skill development.

  • Manage, coordinate, and supplement scheduling in accordance with the individual and company needs.

Staff Coordination and Training:

  • Train new support staff, ensuring they are well-equipped to meet the needs of the residents and in compliance with all 245D required training.

  • Conduct regular staff meetings to discuss program updates, individual progress, and address any concerns or challenges.

  • Provide ongoing training opportunities to enhance staff skills and knowledge in disability services and person-centered care.

Health and Safety Compliance:

  • Ensure compliance with all regulatory guidelines, agency policies, and industry standards for home and community based services.

  • Maintain accurate and up-to-date records, including incident reports, medical records, and individual progress reports.

Advocacy and Communication:

  • Act as a liaison between residents, their families, external service providers, and the organization, fostering effective communication.

  • Advocate for the rights and choices of the residents, promoting self-advocacy whenever possible.

  • Attend meetings and collaborate with interdisciplinary teams to coordinate services and support for the residents.

  • Manage programs that encourage and meet Individualized Home Supports with Training guidelines.

Required Skills:

As a Designated Coordinator, you will organize the delivery of individualized home supports with training, night supervision, and sometimes employment support services to individuals enrolled in a 245D licensed home and community based program according to federal and state regulations, as well as the policies and procedures of the program.

Qualifications

In order to qualify as the Designated Coordinator as defined in Minnesota Statute Chapter 245D.081, you must meet one of the below requirements:

  • A baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older.

  • An associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older.

  • A diploma in a field related to human services, and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older.

  • A minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the requirements identified in (1) to (3) above.

  • Previous experience in a supervisory or management role within the field of developmental disabilities or residential care.

  • Strong understanding of person-centered planning and support methodologies.

  • Excellent communication, problem-solving, and conflict-resolution skills.

  • Ability to work collaboratively with diverse teams and maintain a positive work environment.

  • Proficiency in relevant computer applications for record-keeping, reporting and time card approval/EVV.

Physical Requirements:

  • Ability to lift and carry objects up to 50 pounds.

  • Stamina and physical agility to respond to emergencies and provide assistance when needed.

  • Mobility to navigate the client's home and accompany residents during outings.

  • Visual and auditory acuity to monitor the well-being of residents and ensure their safety.

  • Ability to manage universal sanitary procedures and provide dignified personal cares such as toiliting, showering, and any and all personal cares needed by our clients.
